Do we assume the uniform light coming from above? If in real time there's a light source from the ground, does the AO map still work?

I am curious how do we apply this texture to the model? By adding, multiplying or other method? Should we consider the color of the ambient light or other light sources while applying the AO effect?

Why using texture map leads to a more realistic result? I thought the occlusion map is also computed by the same light source so two rendered images should be the same.
