The transform should be a translation by (1,1) and a scale by (W/2,H/2), without the need for the reflection.

@jfondire -- see the *'d note. The reflection over the x axis make the +y direction point down. I agree that all reasonable coordinate systems increase upward and rightward, but -- unfortunately -- someone chose a bad scanning order during the invention of raster displays and the rest is history.
